Immunization Information for Grades 7 and 12: In accordance with the Iowa vaccine law requirements put in place in 2018, all students entering, advancing, or transferring into 7th grade need proof of one dose of meningococcal vaccine and a DTaP booster.  All students entering, advancing, or transferring into 12th grade will need proof of two doses of meningococcal vaccine; or one dose if received when the student was 16 years of age or older.
