For Preschool/PK: Submit the "Reserve Your Spot Form" first. When we let you know space is available you may submit the forms below to complete your registration. Submit to: St. Joseph Preschool, PO Box 158, Raymond, IA 50667
- Registration Form (if your student was not in our preschool last year)

- Child Health Form (pages 1-2, filled out by parent) each year

ABC AFTER SCHOOL CARE

- $65 family application fee (nonrefundable)

TRANSPORTATION
* Immunization Information for Grades 7 and 12: In accordance with Iowa vaccine laws, all students entering, advancing, or transferring into 7th grade need proof of one dose of meningococcal vaccine and a DTaP booster. All students entering, advancing, or transferring into 12th grade will need proof of two doses of meningococcal vaccine; or one dose if received when the student was 16 years of age or older.
